• Only complete this intake form if a judge has made an order asking the Child and Youth Legal Centre (CYLC) to become involved in your family law case. 
• Submit the completed form within 14 days of receiving.
• Please ensure that all questions in this form are answered. Providing complete information will enable us to make a more timely decision regarding acceptance of the case. 
• Please ask your lawyer to assist you in completing this form if you have one. 
• Please give us adequate time to process the request. We receive many requests, and we process them in the order they arrive at our office. We appreciate your patience. 
• If we accept your child’s case the CYLC will assign a lawyer to represent your child or children.
